Financial Disclosure - Respondent
This is a financial disclosure form which we will use to draft your consent order documents. Both parties will have received an individual disclosure form to fill out and we will collate all the information from both of you.
This form should take you 10-20 minutes to complete. The right-hand side of the form contains instructions to help you answer the questions properly. There is an opportunity on the last page to add any extra information or explanation if needed or to add anything that doesn't fit in the provided boxes.
We do not need proof of any of your bank statements or income unless otherwise requested by your case manager.

About financial disclosure:

You must disclose all assets whether you intend to split them or not. The consent order process requires full and accurate financial disclosure. If you attempt to hide assets or purposely mislead the courts, you may be charged with fraud. Proceedings for contempt of court may be brought against anyone who makes a false statement in a document verified by a statement of truth without an honest belief in its truth. Please note that the consent order process is fully transparent. No financial information can be kept private or hidden from either party; you will be able to see each other’s financial information and addresses in their entirety.
